The specific Home
Health Care services provided are intermittent visits by the following:
MEDICARE COVERAGE
GUIDELINES
Medicare requires that you meet the following criteria:
You must be
homebound.
This means that due to your illness or injury there is an inability to leave
the home and doing so would require a considerable and taxing effort. You
can still be considered homebound if you:

Leave your home
infrequently or for a short duration.

Receive medical
treatment, therapeutic, or psychosocial activities in an accredited adult
day care program.

Leave your home only
to attend religious services.

Leave your home only
to go to the beauty salon or barbershop.

You must have had a
recent
illness or injury,
which requires
skilled nursing,
physical therapy, and/or
speech language pathology on an
intermittent basis, or have a continuing need for
occupational therapy.

You must be
under the care of a
doctor who
has ordered the treatment and services that we are providing. In order for
Medicare to cover the services provided, they must be reasonable and
necessary.

A person with a
psychiatric illness may be considered homebound even if there are no
physical limitations. The nurse will explain specific criteria that apply
to psychiatric diagnoses.
