Ambassador Home Health will been
providing home care services to our community and our promise to
you is:

Providing direct instruction to your office staff on billing for
certification, recertification, and follow-up for your home care
patients.
Ambassador Home
Health’s professionals are available 24 hours a day, 7 days a
week. We consistently have the willingness and availability of
professionals to admit your patients in the evening, on weekends,
and holidays.
Doctors may refer patients to home care upon discharge from an
acute or intermediate care facility. They may also refer a patient
to home care in lieu of hospitalization. All referrals to our
agencies will be evaluated promptly by either sending an
Registered Nurse to visit the hospital to review the patient’s
medical record or coming to your office to do so. We do this to
facilitate the referral process for you, your office staff, or
hospital discharge planners. All that is needed to make a referral
is the patient’s name, current location (hospital or home), and
diagnosis. Or you can complete our
referral
form and Ambassador Home Health will do the rest.
The services we provide include Skilled Nursing, Physical Therapy,
Occupational Therapy, Speech Therapy, Medical Social Workers, and
Home Health Aides Services.
