Providing direct instruction to your office staff on billing for certification, recertification, and follow-up for your home care patients.

Ambassador Home Health’s professionals are available 24 hours a day, 7 days a week. We consistently have the willingness and availability of professionals to admit your patients in the evening, on weekends, and holidays.

Doctors may refer patients to home care upon discharge from an acute or intermediate care facility. They may also refer a patient to home care in lieu of hospitalization. All referrals to our agencies will be evaluated promptly by either sending an Registered Nurse to visit the hospital to review the patient’s medical record or coming to your office to do so. We do this to facilitate the referral process for you, your office staff, or hospital discharge planners. All that is needed to make a referral is the patient’s name, current location (hospital or home), and diagnosis. Or you can complete our referral form and Ambassador Home Health will do the rest.

The services we provide include Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Workers, and Home Health Aides Services.

  • Teaching how to manage a new diagnosis
  • Cardiac Rehabilitation
  • Organizing and teaching medications schedules,
    side effects, and purpose
  • Teaching independent management of diabetes, both insulin and non-insulin dependent
  • Pain management
  • Administering and/or teaching injections including heparin, insulin, Lovenox, Epogen to name a few
  • Wound care
  • Instruction in oxygen therapy and /or nebulizer treatments
  • Teaching on a special diet or fluid restriction
  • Teaching the management of a new ostomy
  • Establishing a home exercise program
  • Strengthening exercises
  • Adaptations for activities of daily living in order to maintain the patient at home
  • Pre-op orthopedic evaluations for home safety
